InsuLearner + Insulearner Web: Nightscout Support

InsuLearner and Insulearner Web now support Nightscout as a data source, in addition to Tidepool. If you use Nightscout to store your pump and CGM data, you can now run the algorithm on it directly. Current version: insulearner==0.2.0.
